Sharing the Care: The Path to Better Outcomes

  • Doug Golub – SVP/General Manager, MediSked
  • Dr. Steve Merahn –  Medical Director, Partners Health Plan/ Care Design New York

Every human has a health resource community composed of a multitude of people – doctors, case managers, providers, families, and brokers. When these people are unaffiliated and operating in silos, it makes the experience of care fragmented. Integrated care requires a single source of truth where all members of a community can benchmark their decisions and actions to create a more concordant care experience. By utilizing health resource community orchestration to support clinical integration and data sharing, organizations can provide better, more person-centered care and improve outcomes for the individuals they support. During this webinar, presenters will share data and case studies from PHP, a provider-led managed care organization that is governed by providers who support people with developmental disabilities dually eligible for Medicaid and Medicare. Presenters will also discuss how artificial intelligence can be used to support integrated care by simplifying the risk stratification process.

Learning Objectives

  1. Understand Health Resource Community Orchestration and how it can be applied in other organizations.
  2. Demonstrate concrete examples of how data was used to improve the integration of care across the multidisciplinary teams ultimately the quality of care and outcomes.
  3. Promote examples of how to measure, monitor, and improve outcomes and quality in the IDD space.
  4. Discuss how artificial intelligence could become a powerful, assistive tool for managed care organizations.
