Sharing the Care: The Path to Better Outcomes

Every human has a health resource community composed of a multitude of people – doctors, case managers, providers, families, and brokers. When these people are unaffiliated and operating in silos, it makes the experience of care fragmented. Integrated care requires a single source of truth where all members of a community can benchmark their decisions and actions to create a more concordant care experience. By utilizing health resource community orchestration to support clinical integration and data sharing, organizations can provide better, more person-centered care and improve outcomes for the individuals they support. During this webinar, presenters will share data and case studies from PHP, a provider-led managed care organization that is governed by providers who support people with developmental disabilities dually eligible for Medicaid and Medicare. Presenters will also discuss how artificial intelligence can be used to support integrated care by simplifying the risk stratification process.

Dr. Steve Merahn is the Medical Director for Partners Health Plan. He is a pediatrician with a deep understanding and experience in the care of children with autism and special needs. He has also served in a variety of physician-executive leadership roles, including Chief Medical Officer at Centria Autism Services and at US Medical Management. A Fellow of the American Academy of Pediatrics and graduate of Albert Einstein College of Medicine in the Bronx with training in pediatrics at Montefiore Medical Center and the Bronx Municipal Hospital Center, Dr. Mehran’s expertise and thought leadership have led him to national speaking engagements covering topics such as healthcare transformation and health equity. He is the author of Care Evolution: Essays on Health Care as a Social Imperative, which advocates for health as a social imperative. He serves as the only physician on the Autism Commission on Quality (ACQ) and is a member of the Primary Care Subcommittee of the American Academy of Pediatrics Section on Developmental and Behavioral Pediatrics (SODBP).

Doug Golub is the SVP/General Manager and co-founder of MediSked focusing his deep HCBS policy expertise and passion on delivering innovative technology solutions to the healthcare and human services field. Golub sits on the ANCOR Foundation Board of Directors and is currently serving as the Board Treasurer. Prior to joining MediSked, Golub served as Platform Services Manager for Microsoft’s Health Solutions Group. Golub holds a Master of Information Systems from Rochester Institute of Technology and a Bachelor of Science in Business Administration and Computer Science from State University of New York College at Geneseo.
