Technology has made its way into most facets of our lives, from smart boards in our schools, to ipad menus at airport restaurants, to personal assistant devices running our homes. The idea behind most of these products is to make life easier and more efficient. In each industry there is a race to create the newest, most innovative products to address certain issues and to ultimately provide the end user with more control.
I noticed that many of my doctor’s offices offer patient portals that are designed to enhance communication, provide a source for records, and assist with the continuity of care. They can be extraordinarily helpful in some cases, to schedule appointments or communicate with care staff, but if you see doctors in different practices then you have to manage multiple portals or deal with providers who don’t offer them at all.
We created MediSked Portal – Person-Centered Portal for the human services world to offer visibility and increase transparency for individuals receiving services. MediSked Portal sits on top of MediSked Connect – Agency Management Platform or MediSked Coordinate – Care Management Platform, and provides an individual and authorized users from their circle of supports and support teams, a secure look into their life. Individuals receiving services have access to their plan, assessments, forms, and budgets; detailed service notes with the ability to approve of not approve logged services, notes, and times; and a secure message center to send and receive messages with their integrated care team.
The Portal drives person-centered planning as it provides the individual receiving services with a platform to direct their life, share their vision and opinion, and gain visibility into their supports. Access to this information is empowering and transparency into the process will work to guide plans and outcomes that reflect the individual’s desired preferences and goals.
Individuals don’t have to deal with the hassle of entering multiple systems to track their services as MediSked Portal supports 3rd party integrations with other data feeds including provider and clinician electronic health record systems, Regional Health Information Organizations (RHIOs), and Health Information Exchanges (HIEs). This flow of information allows for a fuller, more integrated picture of a person’s life that includes real-time information from human and health care service and support providers.
